Problem: Male patient age 59 presented for consultation wanting to restore his mouth to health so that he could smile and chew with confidence again. He had old crowns and bridge work that was failing, which resulted in missing teeth. Due to his business travel schedule, it was important that his treatment be completed in just a few visits.

Patient wanted a whiter smile. She used crest white strips at home. She was not aware that the four crowns on her front teeth would not whiten like her natural teeth. Patient had consulted with her general dentist regarding replacing her four crowns to match her whiter smile. She asked if he could use her existing crowns as the temporaries. He said no, but did not ask her why that would be important to her. When she called our office and asked the same questions, I asked her why that would be important to her. She said that she remembers how awful her temporaries looked and would prefer her existing crowns over awful temporaries again. I assured her that Dr. Djawdan was an excellent cosmetic dentist and that she would love the cosmetic temporaries made by Dr. Djawdan. She was thrilled with her cosmetic temporaries and then more thrilled with her new whiter smile. |

26-year-old male computer engineer presented for consultation. His chief concern was cavities and excessive wear of his teeth. He shared with me that he did not have fluorinated water as a child and as an adult that he drinks a lot of soda and smokes tobacco. The clinical evaluation revealed too many cavities for someone of his age. Also, noted excessive wear on most teeth resulting in loss of anterior guidance. The goal was to restore his teeth to proper health and function. This was achieved by restoring decayed teeth with 8 porcelain inlays, 3 porcelain onlays, and one root canal, build up and porcelain crown. We opened his bite with an orthodontic appliance to allow teeth to erupt to achieve a proper tooth and jaw alignment. Because of his poor bite and jaw misalignment, he had worn down his canines, which are necessary for proper function. We recommended restoring his worn teeth with porcelain veneers. When considering porcelain veneers in cosmetic dentistry, we evaluate the architect of the smile; the relationship between the gums, teeth and facial features. To achieve a more esthetic result, we wanted to address the diastemas (gaps) and the proportion between his gums and teeth. This was achieved with laser gum contouring and 6 porcelain veneers on the upper arch and 8 porcelain veneers on the lower arch. In the first set of veneers we closed the gap between his front teeth completely. After a few weeks of living with his new smile he decided that he missed the space. He asked if we could open the space silently. We firmly believe in doing whatever it takes to make our guests happy with his/her smile, so we redid his upper veneers and opened the space slightly. He was thrilled with his results. |
